Improve User CP settings

The User CP settings require a decent overhaul!

An average user will never be able to use the include/exclude categories options for example.
Those must be a lot more user friendly, eg. <select>-controls showing all categories the users has acces, gving him the possibility to select multiple entries.

Furthermore, I think it is way too confusing to allow selection of both include and exclude filters?

I'd also like to see more placement options, for example to show images within the tab instead of creating its own block.

As a last point, I think the template should be redesigned to look more like vBulletin - currently it stands out which confuses users.
