how do you add a custom field?

I made a custom field, I can't figure out how ot add it. Basically it is an address field. How can I put this near the description area?

I figured out that you could make it required, and it would show up, but I would like ti to only show up on one page, how can i do this?

I would like it so that in one gallery only would it show up.

This is wha tI want to do.
I want someone to be able to pick a file to upload, give the description and keywords. In the gallery of "Through the mail autograph requests" I would like a box where they could post the address. I can add it as a custom field, but then it would show up in all my galleries through the regular upload, and if it is quick upload it would show up on all the galleries, but would require it in areas that I don't want it to require it at.

Currently there is no option to make a custom field appear in only selected categories.
