From what I see guests can not comment thats why you dont see any comments box with captcha

I have guest comments and captcha on, and nothing appears as it should.

Check your usergroup and category permissions as nothing looks like permission is set to comment. So then in turn this is not a captcha question.

How about under admin edit categories?
Did you recently upgrade your forum? For example phpbb2 and phpbb3 are separate integrations. In a normal phpbb3 integration i beleive usergroup 1 is guests so if your forum is different you may not be setting the proper group with permissions.

I also did a "Refresh usergroups from your forum software" under PP admin, but still not working.
What can I do to fix this error ?

Well knowing what usergroup is actually the guest group can help.
You can enable the remaining usergroups allow comments one by one under edit usergroups ( make sure all groups can comment under edit categories ) and then when the comments box appears you know what usergroups is truly guest on your site because it would appear something is not normal there. You can send me a url and admin login as well as ftp info through the contact us link to take a look.

Will mail you the info right away.
