Photos disappear after admin approval

Have a user who has been having problems ever since I upgraded to 5.1 from 4.82. First problem was:

Downloaded an updated version of 5.1, uploaded the files, scanned the database and now the user can upload photos and I can see them when I go into approve them but after approval it doesn't seem to move the photos properly and on the main page it shows the thumbnail and photo missing. When I run a scan on the database it says file not found and deletes it from the records. Look at this page and this is how it appears right after approval and before I run the database scan.

The strangest thing is that I have a basic user account for testing and I have no problems with it.

Found the problem. Users account was corrupted. Had user create a new account and all is working fine now with no problems.
