Help: Header / Footer for VB = Blank page

Hello,
When I try to activate the VB header / footers by editing the header-inc.php file, everything end up becoming a white blank page.

Can you please tell me what the heck im suppose to put for the paths?

The forum is located at "D:\homefolder\user\domain\forum\" and the photopost is located at "D:\homefolder\user\domain\forum\gallery\". What the heck do I use? I've tried several variations of this and nothing is working for me. >.<;;;

If your having trouble with paths view a phpinfo file to check your path.

Also make sure your not deleting anything your not suppose to as you might be causing a parse error and your server is not reporting it onscreen.
