You can only ask additional questions by actually altering the script and template and database because you need to code that up and have database fields for that info etc. It is not as simply as just having the html there.
There is not setting to display the birthday different as its stored in the database in that format.
Actually it is a bit sloppy for a paid script, don't get me wrong , it is a good script. But there should be options for how to display the date and e-mail options like the use of smtp that people can choose. That makes the script more complete and "PRO". Just my 2 cents
