Questions

How do I find what version I'm running?

Also, a moderator accidentally changed a setting, and now on the 'Members Galleries' category, it's showing a thumbnail next to it, however there isn't an option to remove it, how do I remove it?

You would view that photo and hit remove index thumb. They have tagged a photo as the cat avatar.

As far as your version that is listed in your admin panel under statistics.
