How did I endup with two filesizes for the same image?

How did I endup with two filesizes for the same image?

The main image is blurry, but when I click on the second file size, is looks OK.

How can I revert it back to the largest one?

They are not the same size. One is the medium image that your setting in upload options and the other is the large

44.9kb 500x500

93.3kb 600x600

You cant revert back but you can set medium threshold to 0 so no new mediums are created
