Adding rules/requirements on uploadphoto page

On the page where people can upload photos (uploadphoto.php ?) I'd like to add some rules/requirements regarding uploading of photos. I'd like to enter the html (if possible) in a text file and include the text file on that page somehow. How can I do that?

Just edit your uploadphoto template and include a table row with the rules
