Exclamation Help with Hack Attemp!

Someone keeps changing the main page of my site. They put up a page that says... "You have been hacked by Hlec1"

I have no idea how someone could access that and if they can, can they get into my forum and gallery?

What should I do? I have already ftp'd a new index file to remove what they put up. It has happened twice now....

Very worried!
