Save and display medium and large images?
 
Can someone explain exactly what this means?

I am interested in maintaining one image of a reasonable size and tossing the big full-sized versions that users upload.

Is that what this does?

If you turn this off all your site will have is thumbnails. This is not something alot of people do because it would mean large thumbnails.

The easiest way to set things is leave that to yes. Set your thumbnail to 100 and then medium to 0. Set your large settings to the max allowed height and width you want and the uploaded picture will be sized max to your settings and a thumbnail created

And it will then discard the uploaded image if it is 1.5 megs or whatever?

Yes you want the image to be resized down to your settings correct ;)
