Make my own upload form
 

I would like to make my own upload form. I would like a simple form that assigns them to a certain category and gives them the three basic options:

choose image, title, desciption - also need their username

This is for a photo contest and many user say the other form is confusing and hard to use. I don't think it is very hard but if I can make it easy that would be great.
