Views not really counting

I have a strange problem. I have read some threads about views that are not counted, but they dont fit to my problem (or did I miss one?)

One week ago my views counter at http://www.usa-views.com was at 1.018. I can access images as often as I want, from different pcs with different ip addresses (as a not logged in guest) - without any change in the views counter. In the meantime I got 3 new uploads from users, and the views count raised to 1.021. Whats wrong? All caches are disabled, IP cache timeout is set to 1 hour.

Seems it works again. I did some database scans and it started counting again.

There is a view cache that works by caching them and inserting them when the mysql load is low.
