Getting a number of SQL errors

Can someone point me to the right place where I can start debugging?

This has started happening once we turned on a SQUID reverse proxy and added a plugin into vBulletin to detect the real IP address of the visitor (I suspect it is related to one of these two changes).

I would suspect that also. What happens when you turn both off?

The error is because there is already a duplicate there but REPLACE is suppose to be fine with that so try turning off SQUID reverse proxy
