I have seen this happen on an upgrade but not a fresh install.
Do you have a PHP cache software installed ?

well i fixed that ...

It was because of the hack .. vbgallery forumhome, it replaces a file called global_complete.php


the hack has an older version of this file .. so i replaced it with original..
