Hierarchy not working right

You can see on the attached picture how the hierarchy should be.
The problem is that "On the bar" and "In Red" are showing up on the same level as "Taylor" and "Coco", when they should only be (and are) under them.

If I change the Display Order, different sub-categories show up in different areas, in a seemingly random fashion. It has nothing to do with the category or the ordering.

Below is the adv_gallery_categories table. Everything in there looks correct to me.

The hacks that I can remember using are putting the latest image on cmps and playing media inline.

Change the display depth to 1 in the main gallery settings.

Okay... duh... I did that for Main Index, but missed it for Categorybit.

However, that doesn't explain why only some of the sub-categories were showing up.
