how do i set it up so theirs a medium image, so if they click on an image from a cat it goes to the medium image then they click the image again for the fullsize version?

Admin CP => vBa Gallery => Modify Settings => Uploading Options => Save Original Files

it was already set to yes, does that mean i have to set the global sizes down to what i want the medium size to be?

Yes, exactly. If you save your original files then it will save the original without resizing it at all, then your medium sized images will be resized to the dimensions you specify in the 'Edit Filetypes' section.
