Images Not Appearing & Daily Limit Exceeded

On occaision, when a user attempts to upload several pictures, not all of them end up showing up on the site. For example, I attempted to upload 20 images (our daily limit), and only 5 appeared, even after checking back several hours. I tried to go in and upload more, but since I had attempted to upload the limit, I wasn't able to do so.

The other strange thing is that it doesn't always happen. I deleted all of my images, and started over, and had no problems at all.

Anyone have any ideas what may be causing this?

Have you checked the Approval Queue to see if they are awaiting moderation from an admin?

When I go to adm-approve.php, it appears that no images need approving..The message for the Approval Queue category is:

No photos need approval at this time.

I double checked the category to which the user had posted all of his pictures, and only a small portion of them showed up
