Feedback is broken
 

I just got word of it today, and it may have been broken for a very long time, but the feedback system isn't working on my install because the "comment" text edit box isn't available to type any info into. It says "feedback" on one side of the form but the other side, where the text entry box should be, there is a blank table cell. (see attached screen shot)

So, if you click one of the radio boxes and submit the form you get an error because there is no comment.

I guess the JS file contains that portion of the form? I can't find it in the feedback template or the member.php file either.

Where should I look for this part of the form?

Maybe you have altered something?

I visit a profile page and I see a feedback box on my test site the template is profile template.
