Remove category title from ad selection

How can I remove the top category title from ad selection? For instance, someone decides to place an ad and they select Ad type in Step 1:

AD TYPE #1


Then in Step 2, they select the sub category and it displays in the drop down like this:

AD TYPE #1 -> subcategory 1
AD TYPE #1 -> subcategory 2
AD TYPE #1 -> subcategory 3


How can I have the second option in the drop down just show:

subcategory 1
subcategory 2
subcategory 3

And leave the top category out?
