Nothing changed on the site as far as permissions or files, but when adding an image, we are getting this. The data has 777 permissions.

When I scan database and rebuild thumbs, I get this error at the bottom of the page

I would suggest you check that your entire data directory is 777 meaning every last directory especially 29 and the thumbs large directories beneath it.

it was a permission issue. Do permissions get re-set when we run maintenance procedures? That's when it started.

No permissions on directories are never reset in the program. we set permissions on a directory when we create it when a category is created.
