1) Added missing </span> tag in blue.
2) Change the colspan from 2 to 5 to fix cell issue.
3) Remove unnecessary </form> tag in red.

After that page will be XHTML valid too!

I'm trying to get statistics such as "totalimages" on my main website index page via php. I figure i need the code above (or something similar) for me to achieve this?

With other php code I'm using (with SMF forums) this type of code needs a call in the form of "require" at the top of the index page.

Forgive me, I'm a bit rusty with php, hope i made sense?

Your probally better actually creating a discussion topic and seeing is someone can assist instead of trailing on a 3 yr old mod discussion which has nothing to specifically do with what you wish.
