Reporting Tool does not function(FIXED)

When trying to click 'Report Review' in the Reporting Tool, nothing happens except for a page refresh showing the default view. Nothing is sent to the admin (all other emails, contact member links, etc. work fine and are sending).

This is a default install, so no changes have been made to this section.
