This comes from the fact that moderation does not behave like other perms.
If your users, including admin are in more than one usergroup that has: can upload without moderation to NO, then that user cannot upload without moderation.
go to usergroup settings.
Example: You as Admin
if you are in registered users AND in administrator usergroup,
And registered can upload without moderation is NO, you cant upload without moderation even if in admin usergroup this setting is set to yes..
This was the only way to achieve to moderate single users. without making them leave the usergroup they were in.
The best way to do it is:
Set can upload without moderation to yes in ALL usergroups.. (even banned users.. who cares, because if you set: can upload images to NO, they will never get that far.)
Then you create a usergroup called : Moderated where you set that setting to NO:
If you want to moderate a user (because he is regularly posting images you dislike) you just add him to that usergroup.
PS: it could also be that you and your members are in an additional usergroup (like users with more that 10 posts) where moderation is set to no.
OR
You have category permissions set. these would override usergroup settings.
