Can you comment of pictures? If so, do they count as full forum posts?

Also, can you make it where only specific users can upload into a specific folder?

Yes there is a switch to increment post counts for photo uploads and comments. But take note if you run a forum post recount you will lose the photo and comment post counts.

You can make specific usergroups table to post to specific categories yes.
