If the above are correct, then
FTP/FLASH User Folder Path : /home/content/58/6812458/html/forum/gallery/users
FTP/FLASH User Folder URL: h..p://domain.com/forum/gallery/users
The URL always needs h..p://
Make sure the files AND users folders are both chmod 0777
