Shane upgrade documentation is located right in the documentation folder of the downloaded code.
There is an upgrade script you run but all this depends on what version you upgraded from and what else you have or have not done.
Upgrading is usually as simple as uploading the new files minus config files and running an upgrade script.
Running the install script again if you did so incorrectly can definately mess everything up with an integration.
Look what happens when I run your upgrade. Now delete the install and upgrade files off your server.
