Chuck I had to roll back to 5.5 until I can get these 5.6.2 things figured out.
I setup 5.6.2 on my betaforums to show you the issues.
The avatar issue occurs with all members without an avatar in 5.6.2.

5.5 is fine and does not show the red x.

Also I have noticed that you can not view a thumbnail of members who have a personal non public album in 5.6.2
Example on the link below burntire has a thumbnail with 5.5.

Burntire does not have a thumbnail to click in 5.6.2

burntire has a private album on both 5.5 and 5.6.2.
I can view the thumbnails as an admin only.
