Gallery assume that it's installed in the root directory above the ../forum folder. Try changing the double periods ('../') into single periods to see if that works ('./').

To turn off moderation, it's by userroup (ACP --> vBa Gallery --> Usergroup Permissions --> {choose your group} --> Moderate This Group's Images).
