You have not run all your required upgrade scripts.

Looks to me like you never ran the upgrade40?

You working with an older database?

It is still looking for the old vb2 include

try viewing the mysql database settings and change line 41 to vb3

I would double check you have ran all the required upgrades though or you will have other issues.
